Our Support Coordination services are designed to assist individuals with intellectual and developmental disabilities in accessing the care, services, and community resources needed to live as independently and successfully as possible. Our Support Coordinators partner closely with individuals and their families to develop personalized service plans, coordinate providers and supports, monitor progress, and advocate for each person’s needs, preferences, and goals. We also assist individuals in connecting with valuable services such as CLS (Community Living Supports), respite care, speech therapy, ABA therapy, and other specialized supports based on each person’s unique needs. Through ongoing communication, guidance, and person-centered planning, we help enhance safety, independence, community involvement, and overall quality of life.
